The Patient Treatment Card (PTC) is initiated at the point of injury to document essential patient details, observed injuries, vital information, and treatments provided. It serves as a continuous record that remains with the patient throughout their care pathway.

 

  • The PTC and ATMIST cards by Guardians are initiated by the first responder at the point of injury to record essential patient information, mechanism of injury, observed injuries, vital signs, and treatments provided.

    As care progresses, the cards are updated in real time by responders involved in the patient’s management, creating a clear and continuous record of care. This supports decision-making, prioritisation, and effective coordination during evacuation or movement through the medical chain.

    During handover to forward medical care—including paramedics, tactical medics, or clinical facilities—the PTC and ATMIST cards provide a concise, structured overview of the patient’s condition and interventions already performed. This ensures critical information is transferred accurately, reduces duplication of treatment, and supports rapid continuation of care within the forward medical pathway.
